Our SUBSCRIPTION plans are a way to make Health and Wellness AFFORDABLE!
Subscription plans include:
Integrative Health, Hormone Therapy, Fertility, Obstetrics, ADHD and our Wellness program.
*For new patients, there is a one time, non-refundable initiation fee of $100 for any of our subscription plans.
*The subscription plans are NON-Transferrable. And please note that there are no cancellations for one year.
Integrative Health SUBSCRIPTION
Annual subscription with a fee of $150/month for 12 months
($1800 total)
Integrative Health (otherwise known as Integrative Medicine) is a form of medicine that views the body as a complex web of interconnected systems and uses the analysis of all potential factors to determine the cause of an individual’s condition. These factors are comprised of physical, mental, social, and environmental factors.
With this subscription you receive:
- Initial Functional Medicine Assessment
- Monthly check in ( via phone, telehealth, or in person)
- 2 health coaching consults (with our RN/Health Coach)
- Lifestyle Guidance
- Discounts on lab work
- Discounts on supplements
- Portal/text messaging support
- Access to our physician’s videos/webinars
- Discounts on additional programs and Coaching sessions
HOrmone Therapy SUBSCRIPTION
Annual subscription with a fee of $150/month for 12 months
( $1800 total).
With this subscription you receive:
- Initial Hormone Assessment (not just your sex hormones but all hormones)
- Monthly check in ( via text, phone, telehealth, or in person)
- 2 health coaching consults (with our RN/Health Coach)
- Lifestyle Guidance
- Discounts on lab work
- Discounts on supplements
- Portal/text messaging support
- Access to our physician’s videos/webinars
- Discounts on additional programs and Coaching sessions
- You will still need a PCP - Dr. Potter is not considered your PCP
- $100 initiation fee for NEW patients in addition to first month payment
FERTILITY SUBSCRIPTION
Annual subscription with a fee of $150/month for 12 months
( $1800 total).
With this subscription you receive:
- Initial Fertility Assessment
- Monthly to bimonthly check in ( via text, phone, telehealth, or in person)
- 2 health coaching consults (with our RN/Health Coach)
- Lifestyle Guidance
- Discounts on lab work
- Discounts on supplements
- Portal/text messaging support
- Access to our physician’s videos/webinars
- Discounts on additional programs and Coaching sessions
- You will still need a PCP - Dr. Potter is not considered your PCP
- $100 initiation fee for NEW patients in addition to first month payment
OBSTETRIC SUBSCRIPTION
Annual subscription with a fee of $365 per month for 7 months
($2555 total).
With this subscription you receive:
- Routine Prenatal/OB visits
- Postpartum visit
- Initial Labs
- 2 Ultrasounds
Additional Charges: additional Ultrasound, and NST (Non- Stress Test)
Optional labs (not included)
*High risk pregnancies may require additional visits and additional testing. This will require additional fees.
ADHD SUBSCRIPTION
Annual subscription with a fee of $300 per year total.
With this subscription you receive:
- 4 telehealth visits
- ADHD Medication prescription refill for the year
- You will still need a PCP - Dr. Potter is not considered your PCP
wellness program
Annual add on subscription for current subscription patients
with a fee of $50 per month ($600 total)
This is a program/group to make Life Long Changes to your health! We will work together to improve your Metabolic Wellness.
- Instruction on Healthy Lifestyle for life long changes
- Monthly group coaching/meetings
- One on one coaching sessions with our RN/Health Coach
Coaching Sessions
One on one coaching sessions on health and wellness with our Registered Nurse/Health Coach.
This can be bought as a monthly subscription or individual sessions.
